In order to process your Level II Background Check, you need to provide two forms of unexpired identification, one has to be a photo identification, and ORI number.  It is applicant responsibility to provide correct ORI number at appointment time. Applicants may find correct ORI number calling their employer or the licensing agency that regulates the professional activity.
